Sigmund Freud and the Psychoanalysis of Cigars: 20 a Day, More Than 30 Surgeries, and the Truth Behind "Sometimes a Cigar Is Just a Cigar"

In the life of the father of psychoanalysis, there was one dependency he was least willing to have analyzed: cigars. Sigmund Freud (1856-1939) smoked 20 cigars a day, endured more than 30 oral surgeries, and refused medical advice to quit smoking until the final years of his life. He understood the psychological mechanisms of addiction better than anyone, yet showed no intention of resisting his own addiction.

I. A Lifelong Dependency Beginning at Age 24

Freud was born in 1856 in Freiberg, Moravia (now the Czech Republic), and moved with his family to Vienna at the age of 4. After completing his medical education at the University of Vienna, he developed a smoking habit at around age 24. At first, he followed his father's pipe-smoking tradition, but soon switched to cigars.

He regarded cigars as a necessary tool for work, not as entertainment. In his extensive correspondence and diaries, cigars appear repeatedly: "I need to smoke one now before I can organize this thought," and "Without a cigar, I simply cannot work today." At his peak, Freud smoked 20 cigars a day, almost continuously from after breakfast until bedtime.

His father, Jakob Freud, was also a lifelong smoker and lived to the age of 81. This may have given Freud an overly optimistic family expectation regarding the long-term effects of tobacco.

II. Oral Stage Theory and His Own Contradiction

In Freud's psychoanalytic theory, the "Oral Stage" is the first stage of infant psychological development, centered on sensory satisfaction through sucking. If an adult has a fixation in the oral stage, it may manifest as overeating, aggressive speech, or long-term dependence on tobacco.

He understood this logic better than anyone, yet never intended to use it to analyze himself. When students and colleagues mentioned this obvious contradiction, Freud would sometimes conclude with one sentence:

"Sometimes a cigar is just a cigar." (Sometimes a cigar is just a cigar.)

This sentence became one of the most widely circulated Freud quotations of the 20th century. It is also one of the most frequently quoted yet hardest to verify. Scholars generally believe that no exact source for this quotation can be found in his own writings or letters, and that it is very likely a later attribution. Yet precisely because it so perfectly expresses his contradictory character, it has continued to circulate.

III. Oral Cancer Diagnosis: 1923, the Beginning of 16 Years of Torment

In 1923, at the age of 67, Freud was diagnosed with squamous cell carcinoma of the palate. Medical records clearly point to more than 40 years of heavy smoking as the primary cause.

Over the following 16 years, he underwent more than 30 oral surgeries, had most of his jawbone removed, and was fitted with a bulky artificial prosthetic palate. The prosthesis made speaking difficult and eating painful. In private, he called it "the monster."

Doctors repeatedly advised him to quit smoking completely. At most, Freud managed only brief reductions. Each time he tried to quit, he resumed because his mood deteriorated and his work efficiency collapsed. In one letter, he wrote: "Quitting smoking is harder to endure than I imagined. My mood has become unbearably gloomy, and my work suffers as a result. This is harder to bear than the physical pain."

This case was later repeatedly cited by addiction researchers: even someone with a profound understanding of the mechanisms of addiction cannot necessarily escape the shackles of addiction.

IV. The Cigar Ritual in the Vienna Consulting Room

Freud's consulting room at Berggasse 19 in Vienna's ninth district (now the Freud Museum) is one of the most famous spaces in the history of psychoanalysis. Beside the couch covered with Oriental rugs, on the small table behind the doctor's chair, there was always an ashtray and several cigars.

He strictly did not smoke during consultations. This was a professional rule he set for himself. But in the interval between two sessions, he would leave the consulting room, light a cigar, and stand quietly in the corridor until he finished it. Students described this as "the moment when Freud reassembled himself." Here, the cigar was not only a habit, but also a ritual of psychological reset.

V. Exile in London, Cigars in Tow

In 1938, Nazi Germany annexed Austria, and the 82-year-old Freud fled to London with the help of friends. Even in the face of mortal danger, illness, and separation from family, he still brought cigars in his luggage. Collection records from the Freud Museum London state that in his home in Hampstead, cigars remained a constant companion beside his desk.

On September 23, 1939, Freud requested morphine injections in accordance with an arrangement he had made in advance with his physician, and passed away peacefully in his sleep at the age of 83.

VI. An Honest Lesson in Addiction

Freud's cigar story is one of the most honest tests of human willpower. A genius who constructed an entire theoretical framework for "understanding addiction" was equally powerless before his own addiction. This contradiction was not his failure, but a powerful reminder: understanding the mechanisms of addiction and breaking free from the shackles of addiction are two entirely different things.

For the gentleman members of W Cigar Bar, Freud's story offers a completely different entry point into cigar philosophy: cigars are not only sensory enjoyment, but also a mirror reflecting the most honest depths of the human psyche.
